Make a referral

You can get treatment and support in several ways:

  • Refer yourself directly through our form or by phone
  • Ask a family member or friend to refer you through our form or by phone (they must have your permission)
  • Ask your GP to refer you
  • Ask a charity or a health professional to refer you

Complete our online referral form

Our referrals team can be contacted during office hours at gateway@berkshire.nhs.uk or on 0300 365 2000 (press option 4). 

You will be asked to confirm your service number when you refer yourself so please make sure you have this to hand.

We will then arrange a time for you to meet a clinician to have a review to make sure you get the right care and support.

Contact your local Op COURAGE team

You may wish to contact your local team for a range of reasons, such as getting advice about making a referral, letting us know if you need to re-arrange your appointment, or letting us know that you’d like to stop your treatment.

We’re open Monday to Friday, 9am – 5pm.

If you’re not sure whether Op COURAGE is suitable for you or the person you’re thinking of referring, your local team can give you information and advice.

Attending your appointments

We ask that you attend all appointments. If you are unable to attend your appointment, please contact your local team as soon as possible and we will be happy to re-arrange it for you.

If there is anything that we can do to help you attend your appointments, please let us know as early as possible so we can make any adjustments you need.

If Op COURAGE isn’t right for you

There is no obligation to accept the support we offer. We appreciate that you have to be ready mentally to talk and accept support. We’re here as and when you need us.

Please contact your local team and let us know if you would like to stop your treatment so that we can offer your space to someone else.
