Make a referral
You can get treatment and support in several ways:
- Refer yourself directly through our form or by phone
- Ask a family member or friend to refer you through our form or by phone (they must have your permission)
- Ask your GP to refer you
- Ask a charity or a health professional to refer you
Our referrals team can be contacted during office hours at email@example.com or on 0300 365 2000 (press option 4).
You will be asked to confirm your service number when you refer yourself so please make sure you have this to hand.
We will then arrange a time for you to meet a clinician to have a review to make sure you get the right care and support.
